To clarify some of the strange answers given. Yes, you can make a PDF directly from Google Docs as this is a standard feature of ALL word processors The problem is, you want a “fillable” PDF. This can be construed a couple of different ways. In its most basic sense, any PDF can be “filled” using annotations. Just about all PDF viewers, the free ones even, allow you to annotate PDFs. A person can put text in all of the little boxes to their heart’s content. But I suspect you want more functionality than that. You want your user to be able to easily fill out the form, hitting the TAB key from field to field, using checkboxes or radio buttons by clicking them, etc. You want to be able to extract the data t fill and even better, automatically save that data to a table somewhere so you can collect mass information from distributing many of these forms That will take a third-party solution and it will not be free. There are lots of them out there and some of them have been suggested in other answers. The one I have tried is found at PDFFiller.com. It requires a subscription that starts at $8/mo, which is a good price if you have lots of these forms to do on a regular basis.
